WHAT ARE THE REQUIREMENTS: Service Request Form

HOW TO AVAIL OF THE SERVICE :

1 Fill up service request form. Record service request to log book. 2 minutes Customer Service Assistant E   Service Request Form
2 Submit service request form to the Water Maintenance Man C Check and identify whether said complaints are:

  • low water pressure
  • high water pressure
  • no water
  • high chlorine dosage
  • meter leak
  • leak check (Before/After meter)

Leaking Before water meter is responsibility of the water district. Said leaking will be fixed or attended by the water district plumber.

Leaking After the water Meter is no longer the liability of the water district. The concessionaire can hire accredited plumber for its repair. However if they want, they can avail the service of the district’s plumber but they can work it during Saturdays, Sundays and Holidays on their agreed service charge.
